How to Make Salted Caramel Pastry Cream
Step 1: Prepare the milk mixture
In a large saucepan, combine the milk, vanilla, and sugar.
Heat over medium heat until steaming and hot, but not boiling. You should see steam and small bubbles around the edges.
If using a vanilla bean, scrape in the seeds and add the pod.
Set aside and keep warm.
Step 2: Make the caramel
In a separate large, heavy-bottomed saucepan, add the sugar.
Cook over medium heat. The sugar will begin to melt unevenly, form clumps, and then turn into a liquid.

Stir occasionally at first, then more frequently as it melts.
Continue cooking until it reaches a deep amber color. This is where the caramel flavor develops.
